Pivot and Plane Golf Trainer – mid 1980s Need Instructions – Clear Plastic Sheet with lines – mounts on mirror?

This a training aid I bought from a place in Florida. It has lines on it to help determine proper golf pivot and swing plane. I lost the instructions and am not exactly sure how to use. Anyone have the instructions or can help me figure out how to use properly?
